The cornerstone of care management is a thorough assessment of each client from which the Care Manager develops a Comprehensive Care Plan. Meaningful assessment includes the social, medical, psychological, environmental, legal, and financial aspects of a person’s life.
Each Care Plan is designed specifically to help determine the best course of action toward client care, whether in-home care with agency or private services and/or placement in an appropriate facility. When immediate needs are present, GEM staff actively work to meet those initial needs. The Care Plan is thoroughly reviewed with the client, family members and/or responsible parties.
Some clients and their families request only the Assessment and Comprehensive Care Plan to assist them in meeting the client's continued needs. Other clients may need on going professional care management. GEM care coordination services include:
- arranging for, and monitoring services, and social activities and transportation as needed
- home and/or facility visits
- medication oversight
- crisis intervention
- accompanying client during physician/professional visits
- clear and regular communication and reports to family, facility staff, physicians and other professionals
- advocacy
- assistance with transition from home to a care facility, or facility to home
- assistance in filing Long Term Care Insurance claims
- advance care planning
